Additional Forms

 

This page enables parents to download additional student documents and forms to review, outside the portal. A website link for any of the documents as well as a checkbox for parents to confirm they have read the forms may be added.

 

 

 

1. Send your 360° Account Manager the pdf documents that would like to be available for download on this page. Please note that each download link needs to be a separate pdf.

2. Each downloadable document may have a description, an otional hyperlink, and optional checkbox.

3. The instructions for the page, in the first paragraph, can be customized in Tools > Options Setup > Global Settings> Student Options> Web Registration >Additional Forms Instructions.

4.  Select   to submit and continue to the next page.
